** All forms have been redesigned for 2010 ** TIME SENSITIVE: Updated as of May, 2010 with information about Rapps Pharmacy (Not for Dor L'Dor)
Before you start, please make sure you have the latest Adobe Reader. You can download it here if needed.
For each of the relevant* forms below, please take the following steps:
2010 Handbook:- Click on the form. A fillable PDF** will open.
- Fill out the form to the best of your ability. The more information you give us, the better summer experience we can provide for your child.
- Print out the PDF.
- Sign the forms where necessary
